John "Johnny" Abbott IV is son of William "Billy" Abbott and Chelsea Lawson. He was adopted by Billy's then wife Victoria Newman, shortly after birth. Johnny is half-brother to Billy's late daughter Cordelia "Delia" Abbott, Chelsea's twon children, miscarried son Riley Newman, Chelsea's son Connor Newman and adoptive half-brother to Victoria's three children, late daughter Eve Howard II and two living children Reed Hellstrom and Katherine "Katie" Abbott.

Johnny was born on April 9, 2012. Billy and Chelsea had an one night stand when Billy and his wife Victoria Newman briefly split. Chelsea arrives in town a few month later and reveals that she is pregnant with Billy's child. Billy and Victoria had a few failed attempts at having a child as well as a failed adoption.

Billy and Victoria decided that they would raise the child. During her pregnacy Chelsea befriend Victoria's black sheep half-brother Victor "Adam" Newman Jr. Chelsea ends up falling throw a frozen lake and is rescued by Adam. Adam helps delivers Chelsea's son. Chelsea then gives up her rights to her son and Victoria adopts her Johnny.

Billy and Victoria were happily married raising Reed, Delia and Johnny when things all changed when Delia was hit by a car driven by Adam. Billy and Victoria's marriage fell apart following Delia's death. Billy had a one night stand with Kelly Russell which lead Victoria to start dating Kelly's brother Benjamin "Stitch" Russell. Billy and Victoria divorced. Billy and Chelsea started dating while Victoria was pregnant by Billy or Stitch.

Following the divorced, Billy and Victoria decided on joint custody of Johnny. Billy started dating Chelsea and later moved in with Chelsea and Connor. After Victoria gave birth to Katie and then DNA test revealed that Billy was the father.
